Encircling silicone elements, especially the No. 40 silicone band, have become more widely used in the surgery of retinal detachments. The encircling silicone band has been used primarily with underlying silicone implants in order to create a higher buckle and to release the existing vitreous traction. It is the purpose of this article to introduce a spadeshaped needle that is designed to be attached to the No. 40 silicone band, so that the band may be threaded around the eye in a pursestring fashion. By producing small scleral tunnels with the needle through which the band is drawn, the need for anchoring sutures along the length of the band is eliminated. Therefore, the granulomata and infective processes that occasionally result from the anchoring sutures are also eliminated.
