In chronic simple, or "wide-angle," glaucoma the treatment of choice is medical. In the early stages weak miotics should be used and their strength and frequency increased sufficiently to control the intraocular pressure and preserve the visual field. In cases where this is not accomplished satisfactorily some type of filtering operation should be done. The patient should be apprized of this fact and told that the time has come when surgery should be performed. He is told that surgery offers the best chance for the preservation of vision and the visual field for the longest period. Because of the nature of the disease, the uncertainties regarding the final result in all cases of glaucoma should always be explained.
