To the Editor.

—We read Hemady and Foster's¹ letter on anterior chamber cholesterol crystals following hyphema in the January 1990 issue of the ARCHIVES with great interest. Although the finding they describe is extremely rare, cholesterol crystals have been observed previously after bleeding into the anterior chamber. In 1975 we² reported the clinicopathologic findings in eight blind eyes with anterior chamber cholesterolosis and reviewed the world's literature, identifying 88 previously reported cases. One of these reports described the formation of cholesterol crystals in an extensive hyphema caused by blunt trauma 5 days previously.³ In the great majority of cases crystals were found in chronically blind eyes that often had sustained surgical or nonsurgical trauma. Only 3 of 61 patients described in the literature where vision was specified had a visual acuity of 6/60 or better, and 66% had visual acuity of no light perception.
