To the Editor.

—Congratulations on your publication of the recent article by Kushner et al.¹ The prospective, randomized, masked comparison of two different surgical treatments for this condition is a model for clinical strabismus research of this kind.

Data obtained from a less rigorous retrospective analysis of charts in our practice confirm the findings of Kushner and coworkers. Among a series of 30 such patients who received bilateral recessions of the medial recti and posterior fixation sutures using techniques virtually identical to those of Kushner et al, outcome in only 23 was satisfactory according to their category 1 criteria. Six of the 30 patients were significantly exotropic and one was esotropic after surgery. Dr Kushner was kind enough to disclose his methods for calculating augmented recessions of the medial recti for cases of this type several years ago.
