—Bicanalicular silicone intubation is a well-accepted and proven surgical technique for treatment of a variety of pathologic states affecting the lacrimal system, particularly congenital nasolacrimal duct obstruction.1 In some cases, however, it is impossible to perform because of anatomic variations. Most notably, this occurs in partial lacrimal agenesis.

Report of a Case.

—A 3-year-old boy presented with a history of bilateral tearing and mucopurulent discharge occurring since 3 months of age. Conservative treatment with massage and antibiotics was unsuccessful. Bilateral probing was also unsuccessful in relieving the symptoms. An increased tear lake was present on both sides, with mucoid reflux. Both inferior puncta were present and normal. Although superior lacrimal papillae were present, superior puncta were absent. A cutaneous opening of an anlage duct was present on the left.
