In Reply.
—My colleagues and I appreciate Dr Luntz' efforts in bringing his article to our attention. As a matter of fact, his article had originally been included in our reference list, as well as the published discussion of that article by Dr Robert Shaffer. We used this article to indicate our need for developing uniform terminology to describe these relatively rare syndromes. During the many edits that occurred during manuscript development, Dr Luntz' article was inadvertently deleted from the reference list.
We offer our apologies to Dr Luntz for the oversight. His work in this area has been important and of value. Indeed, he and I have personally reviewed photographs in order to help standardize our own thinking in these areas.
My colleagues and I continue to prefer the anatomical classification, because it more specifically describes what is seen by the examining surgeon and, therefore, provides more precise communications
