Having recently immigrated to the United States from Canada, where a national health scheme has been in operation since 1969, I consider the present efforts of the US government regarding health care as a "déjà vu" of exquisite clarity.

The recent passage of the Deficit Reduction Act has mandated the Health Care Financing Administration (HCFA) to freeze Medicare fees and offer physicians the choice of whether or not to accept these fees as full payment. A roster of participating physicians is to be published and made available to the public. It is possible that fees will be increased after 15 months. Physicians not entering the plan will not be on the list, and their fee structure will not be reviewed. Two thirds of the 3,266 respondents to a recent American Academy of Ophthalmology survey were not going to accept the HCFA offer.
