We read with interest the article by Heindl et al1 regarding histologic and transmission electron microscopy analysis of 20 Descemet membrane specimens obtained after Descemet stripping in Descemet-stripping automated endothelial keratoplasty (DSAEK) and would like to congratulate the authors for the new insight gained by their assessment of Descemet membrane specimens. However, we have a few questions concerning the methodology of the study.
1. There are several techniques for inserting a donor posterior lamellar disc in DSAEK. The authors describe an unusual approach of using an 8-mm nasal scleral tunnel that has not been described in the literature. One might think creation of such an incision is not only surgically difficult but may create a potential risk of pseudopterygium and/or induced astigmatism. Even for pull-through techniques, the literature suggests using smaller (usually 5 mm) scleral, limbal, and/or clear corneal incisions superiorly or . . .
