We appreciate the comments by Essex and colleagues on our study. As a general rule, cases and control subjects should be treated similarly in randomized clinical trials, which is the reason we injected BSS (as the safest placebo) for control subjects vs intraocular antibiotics for cases. Injection with BSS is used in everyday practice during different types of intraocular surgery, and we have not encountered any report demonstrating that sterile intraocular injection of BSS per se could lead to endophthalmitis. Essex and colleagues are well aware that during repair of an open injury, reformation of the globe with BSS may be required several times. To determine whether intraocular injection of BSS during primary repair increases the rate of posttraumatic endophthalmitis, we have compared the incidence of endophthalmitis in 167 control eyes . . .
