Although it is anecdotal and as far as we know not published in the literature, we have made the same observation as Drs Yuen, Cheng, and Chan that removal of the eyelid speculum at the end of surgery may result in ingress of ocular surface fluid into the anterior chamber. On the other hand, benefits of eye patching after cataract surgery have been a source of controversy. Some authors propose that it increases intraocular inflammation,1 while others indicate that not patching may be associated with higher risk of endophthalmitis.2 However, various confounding factors associated with patching have to be analyzed in order to weigh its real participation in ocular surface fluid inflow. The pros and cons of patching the eye in the early postoperative period require a more rigorous scrutiny in order to establish final recommendations.
