We read with great interest the article by Azuara-Blanco et al¹ in the March issue of the ARCHIVES regarding the creation of a pseudocapsulorrhexis in a case of Chandler syndrome with ectopic basement membrane–like material. However, we would like to point out that a "pseudo" or partially split capsulorrhexis has been described by splitting of a normal lens capsule during the procedure, giving rise to the "double-ring" sign after a complete or partial capsulorrhexis.² The presence of a double ring sign may necessitate a deeper puncture of the partially split anterior capsule to complete the anterior capsulorrhexis for hydrodissection and cataract extraction (Kevin Sullivan, MD, written communication, November 1997).

It is important to recognize the creation of "pseudo" or incomplete anterior capsulorrhexis in normal lens capsules, as well as in eyes with ectopic basement membrane or fibrous material on the anterior lens surface, to prevent complications during cataract surgery.
