I read with interest the article by Koenig and Mieler,¹ which was published in the September 1996 issue of the ARCHIVES.

The surgical handling of the subluxated lens in Marfan syndrome and the correction of aphakia by the implantation of an anterior chamber intraocular lens (IOL) as described by these authors may be hazardous. This article disregards some of the fundamental problems associated with the surgery of ectopia lentis and those associated with the implantation of anterior chamber lenses in young patients. Moreover, this article fails to address the potential pitfalls associated with a follow-up of only a few months for 4 patients who underwent these procedures.

The authors suggest removing "vitreous around the dislocated lens" before cutting (in young patients) or fragmenting (in older patients) and aspirating the lens.
