A 50-year-old woman was evaluated for a subretinal hemorrhage that extended
into the vitreous of the right eye. Examination showed multiple refractile
brown spherules in the vitreous. A vitrectomy was performed, and pathologic
examination of the spherules showed them to be composed of free hemoglobin.
Free-hemoglobin spherulosis in the vitreous, like cholesterosis bulbi, is a
manifestation of vitreous hemorrhage.
